Any biological samples provided will also be destroyed as soon as reasonably practicable and will only be used for the purposes you have consented to.

We collect personal data as part of our booking and testing process. Registration is required for all tests and reporting of results. It is essential that all information is completed accurately and that you confirm your acceptance of this policy.

• Unique Registration Number (URN) from your test kit (this must be recorded accurately to match your sample correctly)
• Full name
• Date of birth
• Biological sex
• Email address
• Country / nationality
• Telephone number
• Date the sample was collected
• Full home address (including postcode)

We only collect information that is necessary. Any third parties involved in delivering the service will only have access to the data required and will be bound by the same data protection obligations.

Other Limitations

Medical practitioners may reasonably hold differing clinical opinions regarding the same condition or symptoms. Such differences do not necessarily indicate that our services are defective. Proactive Health and Wellbeing is not liable for the opinions provided by medical practitioners.

We maintain a zero-tolerance policy towards abusive or offensive behaviour directed at our staff.
